Requirements
- Licensed in the state of Illinois as a Licensed Social Worker
- Master’s Degree in Social Work is required
- Basic Life Support is required
Responsibilities
- Help patients and their families navigate the medical system
- Assess and monitor patients’ and family members’ mental and emotional health
- Provide short term counseling and therapy
- Communicate patient needs and concerns to the larger medical team
Preferred Qualifications
- 3 years of discharge planning is preferred
- 3 years in social service field is preferred
