Requirements
- Bachelor’s degree in Nursing required
- Current Ohio Registered Nurse licensure required
- 3 years of relevant experience required
- Successful completion of a background check required
- Selected candidate may be requested to complete a pre-employment physical including a drug screen
Responsibilities
- Coordinate and efficiently operate the MOMI PODS program
- Organize and direct activities and services provided by the program
- Develop and implement program goals and objectives
- Evaluate program progress and outcomes and identify quality improvement areas
- Plan, manage, and evaluate MOMI PODS reporting, data management, education, and outreach programs and activities
- Contribute to budgeting and resource allocation to meet objectives of the Care and Innovation Community Improvement Plan (CICIP)
- Respond to and ensure adherence to program funder requirements
- Prepare comprehensive quarterly and year-end progress reports
- Prepare and disseminate annual MOMI PODS report for division, department, and medical center administration
- Provide ongoing assessment of patients and their families regarding treatment, complications, and general health maintenance
- Develop and provide educational programs, materials, and information
- Monitor and report safety issues
- Serve as a resource for care management for patients, referring physicians, and specialty services and staff
- Prepare presentations and provide education to professional health care providers
- Serve as an advocate for appropriate patient care with insurance carriers, professional providers of care, legislators, and other entities
