Requirements
- Licensed Master Social Worker in the state of South Carolina
- Master’s degree in a social work or social welfare program accredited by the Council on Social Work Education
- MSW licensure by the South Carolina Board of Social Work Examiners
Responsibilities
- Provides continuum of care for patients from admission to discharge
- Assesses social, emotional, and financial needs of the patient
- Assists in developing discharge plans with patients and their families
- Referrals to community resources and/or placement in an appropriate facility, following discharge
Preferred Qualifications
- Certification in case management preferred
